MassJam We will be leaving for MassJam at 5:30 AM on Saturday. If you have not filled out a medical form, here it is The good news: For individuals who do not have a medical form (Class 3), have them complete the front of the form ONLY - sections I, II, III, IV, and VI. They DO NOT have to go to a doctor for a physical - section V does not need to be filled out. However, it is absolutely necessary for the Parental Statement/Permission to Treat (section III) to be signed and dated. If you are bringing your medical form on Saturday, you MUST bring 3 signed and dated copies, no exceptions. Get your permission slip
